We are now scheduling outdoor assisted living and skilled nursing visits at our facilities

Please see below for the visitation guidelines for each St. Luke facility:

NORTH CANTON FACILITY
*Visitation temporarily suspended effective 8/29/2020 until further notice

What You Need To Know:

  • Prescheduled visitation is available 7 days per week from the hours of 10am to 4p for a maximum of 30 minutes per day.
  • Visits must be scheduled at least 24 hours in advance for Monday-Friday visits and at least 48 hours in advance for Saturday and Sunday visits.
  • Visits will be limited to no more than 3 visitors per visit; each resident is permitted 3 in person visits/week.
  • No children under the age of 2 will be permitted at this time, all others must be capable by age/maturity to maintain social distancing and wear a mask.
  • No food or drink is permitted to be consumed during the visit. If you would like to bring your loved one a food or drink, it must be dropped off at the front desk and it will be delivered to their room for after the visit.
  • The facility reserves the right to reschedule visitation due to inclement weather, including extreme heat.

For ease of scheduling a visit, we ask that you please call 330-499-8341 and select #3, leave a message with your name and phone number and someone will return your call within 24 hours.

Guidelines:

Please arrive 10 minutes prior to your scheduled visit.

Once you arrive to the main entrance, proceed to the front desk for screening.

1. Each visitor will be required to perform hand hygiene prior to, during as needed, and after each visit and wear a face mask. Visitors must provide their own face mask.

2. Your temperature will be taken and you will be asked to complete a screening form

     a. Visitors will be screened for fever and respiratory illness using a designated form prior to entry.  
         Those with symptoms of a respiratory illness (fever, cough, shortness of breath, or sore throat) will
         not be permitted to visit.

     b. Sign in is required in the designated visitor log book.

3. Once screening is completed, staff will direct you to the designated area.

4. Visitors are limited to an outside patio designated by the facility, please do not rearrange items.

5. Refrain from physical contact with resident and others while visiting the facility. Example:  practice social distancing with no hand shaking or hugging and remaining 6 feet apart.

6. At the prescheduled time we will assist your loved one to the designated area. Residents are required to wear a mask during the visit.

7. Visit is limited to 30 minutes, once you are at your designated area, if you would like a shorter visit or need to leave early, please call 330-499-8341 and ask for your loved one to be assisted back inside. Please do not leave without making sure your loved one is returned safely inside.

PORTAGE LAKES FACILITY

What You Need To Know:

  • Prescheduled visitation is available Monday, Wednesday, Saturday from the hours of 10am to 11am and 1pm to 3pm for a maximum of 30 minutes per day.
  • Visits must be scheduled at least 24 hours in advance for Monday-Friday visits and at least 48 hours in advance for Saturday and Sunday visits. Please be sure to call during business hours 9aa-5pm.
  • Visits will be limited to no more than 3 visitors per visit; each resident is permitted 2 in person visits/week.
  • No children under the age of 10 will be permitted at this time, all others must be capable by age/maturity to maintain social distancing and wear a mask.
  • No pets allowed.
  • No food or drink is permitted to be consumed during the visit. If you would like to bring your loved one a food or drink, it must be dropped off at the front door and it will be delivered to their room for after the visit.
  • The facility reserves the right to reschedule visitation for any reason including inclement weather, extreme heat, etc.
